Job Description

Role and Responsibilities

Vitalyze is a unique Value Based Healthcare platform company that delivers on-demand Value Based Clinical and Population Interventions for healthcare partners driving optimal quality, satisfaction and revenue. As a health coach you are the care coordinator for our patients. We are seeking experienced healthcare professionals that can provide health coaching and care coordination services for our clients through our healthcare-as-a-service platform.

·       Virtual initial intakes (data entry on lifestyle habits, medical and family history)

·       Actively communicate with patients throughout their experience

·       Work with our MU2 Certified EMR

·       Administrative and logistical work for patients

·       Content creation and input on our process

·       Ongoing coaching as needed

Qualifications and Education Requirements

MA, LPN, RN, RD, CHES, Exercise Physiologist, or other allied health professionals

1-3 years experience in a clinical health coaching or care coordination role

Health coaching certification - Wellcoaches preferred, but willing to consider other certifications

Experience in motivational interviewing, behavior change techniques, goal setting, care coordination

Familiar with medical setting including working with physicians, nurse practitioners, allied health professionals

Basic medical terminology, disease management and chronic care management experience preferred

Proficient with technology (video conferencing, email, text, shared documents, etc.)

Personable individual and good relationship builder

Passionate about health and wellness

Preferred Skills

Well developed communication skills

Familiar with terminology related to medical coding (ICD 10), basic laboratory tests (eg. Diabetes screening), Familiarity with working in Disease Management, General Preventive Medicine or Population Health

Clinical Experience with Chronic Disease Management (eg. Diabetes Prevention, Cardiovascular Disease, etc) preferred but not required

Experience with an MU 2 Compliant EMR

Competent problem solver

Independent self-starter and collaborative team player
