Youth & Family Specialist (Child Crisis Hospital Team)

Job Description

The Child Crisis Hospital Team Youth & Family Specialist will provide temporary in home support to the child/family and facilitating a smooth transition of care to the responsible clinical home for ongoing support and clinical intervention. These services will be provided to children and families who present at any hospital in Maricopa County in crisis and the children do not meet the criteria for hospital admission but require further intervention to remain in the community. Provide ongoing care coordination services to clients and families. Perform out of office care coordination; provide home or community-based therapeutic and supportive services in accordance with agency procedures.  Perform administrative tasks in accordance with agency procedures.

Responsibilities:

  • Provide comprehensive support to consumers and their families through in-home and community based interventions and education to address the consumer’s therapeutic goals. This may include parent education and skills training, behavior
    management training, school observations, consultations with school personnel,living skills training, and social skills training.

  • Serve as liaison between client, family and ongoing service provider, completes all documentation in a timely manner.  Perform necessary tasks to help families understand how a crisis can impact the entire family system, help the family to
    identify and create a natural support system. Performs necessary tasks to support the client and family, which may
    include outreach and engagement efforts.
  • Perform ongoing case coordination services to children with serious behavioral health issues and their families in accordance with the Child and Family Team process and the Behavioral Health Service Plan. Assist clients and families in obtaining community support, identifying resources and resolving barriers. Support the most appropriate and least restrictive levels of care.   Complete all appropriate documentation, including contact notes, BHSP, SN&CD, Crisis Plans, OOH packets, SCA, ASR, etc.
  • Provide ongoing services to ensure the coordination of behavioral health services with other agencies, such as Child
    Protective Services, Probation and Parole Departments, Division of Developmental Disabilities, Arizona Long Term Care, schools and other providers.
  • Work collaboratively with the client and family to prioritize and record the family vision and goals via the Behavioral Health
    Service Plan, Safety Plan, and all other related clinical documentation.

Qualifications:

Behavioral health experience; Inpatient/Residential experience; Crisis and/or hospital setting or Group Home/DDD/CPS in-home work experience required.

 

Bachelor’s Degree in a behavioral health related field; or a Bachelor’s in any discipline with at least one year full-time direct behavioral health work experience; Associate’s Degree with at least two (2) years full-time direct behavioral health work experience; or a High School Diploma with four (4) years full time direct behavioral health work experience.

 

Spanish or Arabic Bilingual preferred
